- Must be able to train at our main office in Arlington Heights for up to 2 weeks depending on your experience. .
- This is a Home Health position where you will be making a 45 minute visit at the patient’s home.

Position Summary:
A Home Health Registered Nurse (Case Manager) is responsible for traveling to a patient’s home to provide multidimensional care, administer their services and helping patients maintain their independence.
We are seeking an individual who is available for a Per-Visit Position. We are requiring at least 5 home based per week visits. Home Health Nurse creates care plans to achieve goals based on the patient's diagnosis and needs.
Some of the duties include administering at-home IVs, changing dressings, cleaning wounds and periodically updating Doctors about their patient’s health.
Responsibilities:
- Assessing the patient's physical, psychological, and social needs.
- Chart observations of patient's condition at each visit.
- Developing an individualized care plan for each patient and providing care that is in accordance with the plan.
- Teaching basic self-care skills to patients or their caregivers to help them manage chronic conditions at home.
- Medication teaching/management
- Wound care, IV management, PICC Line care, G-Tube care, Foley care
- Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
- Recommend tools or devices that may improve the quality of life for the patient.
Qualifications:
- At a minimum, an Associate's Degree in Nursing, or a Bachelor of Science degree
- Prior Home Health experience and the knowledge of Home Health documentation IS REQUIRED
- Ability to work independently or collaboratively with physician or therapeutic team
- Knowledge of Home Health care guidelines and standards
- Flexibility and patience to work in a variety of living environments
- Ability to remain calm under pressure
